For Covid-19 vaccination of a minor, download the COVID Vaccine Parental Consent Form or complete it in Skyward.
If you or your child is coming in for Covid testing, please download and complete the Covid Testing Consent Form. Bring the completed form to your appointment or arrive a few minutes early to fill one out there.
