Covid-19 Vaccination
Moderna and Pfizer vaccines are in stock at all clinics. Minors need a Teen Health Parental Consent Form (which GISD families completed during school registration) and a Covid-19 Vaccine Consent Form completed by a parent or guardian. The parent or child may then make an appointment at the Teen Health clinic of their choice.
Covid-19 Testing
Any GISD student or staff member can make an appointment at a Teen Health clinic and get test results in 15 minutes, free of charge. GISD students and staff are welcome to make an appointment at clinics with direct exterior entrances: Collegiate Academy, Central Middle School, or Austin Middle School. Appointments will be scheduled as soon as possible, depending on availability. Ball High and MECC students and staff who become symptomatic at school may be seen at the clinic on their home campus, also depending on availability. To schedule an appointment, call 409-761-3530.
To receive a rapid COVID-19 test, a consent form must be completed by a parent or guardian. To expedite this process, we are asking all parents to log onto Skyward Family Access, go to online forms, and complete the COVID-19 Consent Form. You may also download the form here.
